The talent release form for my FMP this year:
This is the talent release form which is used to get the permission of the actor so that they can be in your production. The talent release form also has the actor's name, phone number, email, their manager/agent and their phone number, the producer's name, the name of the production company, the name of the production, the compensation being given and any additional terms and conditions included as well. The sheet also has a section where the legal guardian or talent sign to legally agree to being a part of the production. The talent's name for my music video that I am creating for my FMP is Daniel Smith, with his email and phone number underneath his name on the form.
The producer of the music video is me, Samuel Pattison, the production company is called Chromatic Productions and the production title is 'Unknown Mortal Orchestra- Swim and Sleep (Like a Shark) Music Video'. The additional terms and conditions state that their face may be unrecognizable during parts of the music video due to the distortion video effect being added so they have to be fine with that happening to star in it and there is also a signature done by the talent with the date they signed the talent release form agreeing to be in my FMP next to their signature which are both at the bottom of the page.
