Disconnected Risk Assessment Form
This is the risk assessment form for my video installation Disconnected and its purpose is to state any risks that are present in a location that is being filmed in, who is at risk of being possibly injured or worse (cast/crew), what property might be damaged during filming as a result of the risk causing an accident, what risk controls are already in place, the level of risk (LOW, MODERATE, HIGH, EXTREME) and any further action required to control a risk. It is basically what keeps the location that is being filmed in safe and void of any possible injuries, casualties or deaths in the worse case scenario as well as any camera equipment being damaged or destroyed.
