Disconnected Talent Release Form
This is the talent release form for my video installation Disconnected which includes a signature from myself because I am the one starring in my video installation and there is only one form because I am the only one starring in it. The purpose of the talent release form is to receive the talent's permission legally to take part in a production and if the talent is below the age of 18 there is the space at the bottom for their legal guardian to sign and give permission as well but because I am above the age of 18 and an adult a signature in that part will not be necessary.
